As a new generation medical device company, Applied Medical is committed to supporting independent clinical research and scientific studies in an effort to advance minimally invasive surgery and patient outcomes.
To have a request for support formally considered by Applied Medical, please complete the following clinical research questionnaire. You will be contacted within 30 days from submission by our Clinical Research Committee.

A. APPLIED MEDICAL IS NOT OBLIGATED TO KEEP YOUR SUBMISSION CONFIDENTIAL. You understand and agree that Applied Medical is not obligated to keep as confidential any information concerning your submission. If the non-confidential disclosure of your submission could subject you to any kind of losses, claims, or damages, please do not submit your request for research support to Applied Medical.
B. YOU WILL RECEIVE NO COMPENSATION FOR SUBMITTING DETAILS ABOUT YOUR PLANNED RESEARCH. You understand and agree that no agreement for compensation is, or shall be implied by the submission, receipt, consideration, evaluation or use of your information or idea(s) unless expressly agreed to in a separate written agreement executed by an authorized representative of Applied Medical.
C. APPLIED MEDICAL IS FREE TO USE YOUR SUBMISSION. You understand and agree that Applied Medical assumes no obligation with respect to any submitted information. Applied Medical shall be free to use, copy, modify, disclose and distribute any information or idea in your submission for any purpose without restriction. Applied Medical’s review of your submission, with or without any subsequent entry into any discussions or negotiations with you, shall not be deemed an admission as to the novelty, nonobviousness, priority or ownership of your submission, nor will such review in any way impair Applied Medical’s right to contest the validity or any alleged infringement of your rights, if any, to that submission.
D. APPLIED MEDICAL IS UNDER NO OBLIGATION TO REVIEW OR APPROVE YOUR REQUEST FOR SUPPORT FOR RESEARCH. You understand and agree that Applied Medical is under no obligation to review or approve support for your submitted request.
E. YOU MUST CERTIFY THAT YOU LEGALLY HAVE THE RIGHT TO DISCLOSE THE SUBMISSION. You hereby certify that you legally have the right to disclose information pertaining to the planned study, and that you are not obligated by any other agreement to keep information confidential, and that you are at least 18 years old.
F. NO RELATIONSHIP FORMED. You understand and agree that no confidential, consulting, or other relationship exists or is established, expressed or implied, between you and Applied Medical by submitting your request for support of research.
